A useful expense form for employees and contractors - ideal for remote workers...
This scaffold creates a simple employee/contractor monthly expense sheet with seperate sections for general expenses and mileage. It also contains an authorisation section at the bottom of the form which will only be visible to managers, etc.
Each page created with the form will cover an unlimited number of expenses for one employee/contractor for one month. At the end of the month, simply restrict page editing to a manager group or move in to your accounts space.

Download scaffold source as a text file (There are lots of very long lines and after trying to embed it on this page in {noformat} tags, I decided the vertical scrolling was way too big so I put the wiki notation in to a text file instead!)
You would likely need to change the group used in the {show-to} macro (near the bottom of the scaffold) to whatever groups you already have defined.
