Yeah, we had to remove the logout links and found that this could only be done by editing that macros.vm file (as well as the decorator files). Also, we found that when using an external authenticator, you need to make a decision around the "password" tab that appears for users profile and what you want to appear there.  
 
There are some discussions on this forum as to various approaches if you do a search.

          you enable the 'External user management' setting in "Administration Console Configuration General Configuration", Confluence will not display the Change Password tab in the users profile. Regards
