Some JIRA icons missing

Problem
Icons are randomly missing (invisible) in JIRA e.g. delete icon for comments, or icons in the wiki editor for issue description, comments etc. See sample screenshots below:
Diagnosis
- Problem occurs consistently with all web browsers, and from all client machines
- Problem occurs when accessing JIRA with or without a web proxy.
Cause
These icons are loaded from tomcat cache located in the $JIRA-installation-directory/work folder. The tomcat file cache may be corrupt for some reason.
Resolution
Restarting JIRA should force a re-build of the tomcat cache and resolve this problem.
