Dashboard gadgets display Forbidden Error

Problem
When you load a Jira Dashboard, all the Gadgets in the Dashboard display a Forbidden Error:
Environment
Jira Server/Data Center on any version from 7.0.0.
Cause
Jira is running behind an Apache load balancer or reverse proxy server which has mod_evasive enabled.
Resolution
Disable mod_evasive and reload Apache's configuration.
