Smart commits are not creating reviews
Problem
The following appears in the atlassian-fisheye.log:

java.lang.RuntimeException: java.lang.RuntimeException: com.cenqua.crucible.revision.source.SourceException: Repository <REPO_NAME> is not available:Cause
The user running the smart commit does not have proper permissions to the repository in Crucible.
Resolution
Go to Administration > Repositories > REPO_NAME > Permissions and make sure this user is in one of the groups with a check mark next to it.
