Confluence 7.4.9 contains a fix for an unknown attachments issue CONFSERVER-55928 - Getting issue details... STATUS . This fix will prevent the problem from happening in future. We're still working on a fix to rectify the problem on existing pages. If you're running 7.4 LTS, we recommend upgrading to 7.4.9 to get this fix.
If you're still running 6.13 LTS we recommend you don't upgrade to 7.4.x just yet, as a content duplication issue CONFSERVER-59227 - Getting issue details... STATUS could impact your site. We're working on it right now, and recommend waiting until the fix is available before upgrading to 7.4 LTS.
